According to a poster on Lugnet, this may have been sold at Costco or other warehouse stores in the USA.

This promo polybag was given away at US Toys R Us stores on October 27, 2012. If you helped build vignettes for the store's "graveyard scene" (by sorting through a bulk lot of bricks and following building instructions on paper, all supplied by the store), then upon completion you received this polybag.
(Or, if you did what I did and just asked nicely, the friendly supervisor gave you a copy...)

@Huw, how do the barcode images get created, are they actually just images, or is there some clever trickery at work that auto-gens them?
The barcode images are images generated by some 'clever trickery' and it looks as if they have been.
^^ #5000281 doesn't have a barcode on the bag itself; there's only one on the preorder card it's originally attached to (which is what they ring up when you buy it).

On the back of the box, they list the elements, and the clown is listed as a single piece. This lead me to ask an associate at the LEGO store if the minifig was glued, but they said it wasn't (and that's correct). It's a really nice set, but a bit pricey.
